A Nurse-Led Approach to Health Equity

People with learning disabilities face real health inequalities, and their symptoms are too often missed or put down to something else. Our nurse-led team works to close that gap by supporting annual health checks, keeping health action plans up to date, preparing hospital passports, and helping people communicate during appointments.

We work in genuine partnership with families, support coordinators, occupational therapists, speech and language therapists, and local authority teams. Agreed strategies are followed consistently, and everyone involved is kept informed through accessible, regular reporting.
