Learn moreComplex needs can change how care is delivered. They should not change how care feels. We are independently owned and nurse-led, which means fewer layers between the person receiving care, the team delivering it and the people responsible for the decisions.
Care plans receive nursing review and sign-off where our clinical governance process requires it. Where needs are complex, clinical or higher risk, nursing assessment and oversight form part of the care pathway.
Care can be delivered around the clock where that forms part of the agreed package, with an out-of-hours escalation route available to clients, families and care teams so concerns can be raised quickly.
We aim to build a small, consistent team around each person — not only their clinical needs, but their routines, communication and what matters to them. We cannot promise the same individual at every visit, but we work to keep changes to a minimum and tell families when the team changes.
Our clinical and operational leadership stays close to care delivery, escalation, safeguarding and changes in need, so families and professionals know who is responsible and can reach them.

You do not need to know what type of care you need before you call. Most people don’t.
Tell a Care Coordinator what is happening, what support is already in place and what is worrying you. We will help you work out what the next step actually is. If we identify clinical or complex needs, a nursing assessment and clinical review become part of that process.
Talk to our care teamFor case managers, discharge teams, commissioners, social workers and other health and care professionals.
Send us the key information and we will consider the clinical needs, required interventions, staffing and competencies, care hours, proposed start or discharge date, funding route, and whether we can safely mobilise and sustain the package.
Clinical acceptance is never automatic. Final acceptance of complex packages depends on clinical assessment, workforce readiness and safe mobilisation. Urgent professional referrals are prioritised for prompt review.
Many people are entitled to financial support for home care through their local authority, NHS Continuing Healthcare, or direct payments. We can help you work out which options apply to you.
