Background
One of our clients lives with diabetes and experiences mobility difficulties, which increased her risk of falls, reduced independence, and the development of pressure sores. She required ongoing support to maintain her wellbeing while continuing to live safely and comfortably at home.
The Challenge
Due to limited mobility, the client was at risk of becoming less active, which could have negatively affected both her physical health and diabetic condition. There were also concerns regarding skin integrity and the possibility of pressure sores developing if appropriate preventative measures were not maintained.
Our Intervention
Our care team implemented a person-centred approach focused on safety, wellbeing, and independence. This included:
Encouraging and supporting regular mobility to help maintain strength and reduce the risk of falls.
Promoting simple exercises and repositioning techniques to help prevent pressure sores.
Monitoring and observing skin integrity during care visits.
Immediately reporting and escalating concerns when pressure sores or red areas were identified to the appropriate healthcare professionals and family members.
Monitoring blood sugar levels and observing for any signs or symptoms that could indicate instability in her diabetic condition.
Encouraging the client to remain as independent as possible with daily activities, while providing support where needed.
Maintaining consistent communication between our agency, the client, and her daughters to ensure continuity of care and collaborative decision-making.
The Outcome
As a result of the support provided:
The client became more engaged in mobility and daily activities.
Risks associated with falls and pressure sores were reduced through early intervention and preventative care.
Any concerns identified were addressed promptly through effective communication and escalation procedures.
The client and her family felt reassured knowing that her condition was being closely monitored.
